Register for Patient Services app

Use this service to register for Patient Services to request medication from the practice.

You can use this service if you:

• are registered at the surgery

Before you start

We’ll ask you for:

• your first and last name, date of birth, sex, postcode, email and phone number
• if applicable, the details of the person you are completing the form on behalf of
• two forms of identification (one photograph ID and one proof of address)

What happens next

Once we have registered you, we will email you a letter containing your:

• Practice ID
• Account ID
• Linkage Key

You only need these details to set up your account. After activation, you will simply log in using your account details.
