Under the Data Protection Act 2018, you have a legal right to access your health records. If you want to see your health records, please complete our contact the practice form. You don’t have to give a reason for wanting to see your records.
It’s a good idea to state the dates of the records that you want to see; for example, from 2009-2012. The practice has 30 days to respond to all requests. You will be asked to provide photographic identification to obtain your medical records. The requested medical records will be sent to you via email in an encrypted and password-protected file.
Hospital records
To see your hospital health records, you will have to contact your local hospital. Your request to see your records will be forwarded to the health records manager.
Power of attorney
Your health records are confidential, and members of your family are not allowed to see them, unless you give them written permission, or they have power of attorney and it is active. A lasting power of attorney is a legal document that allows you to appoint someone to make decisions for you, should you become incapable of making decisions yourself.
