My diabetes, My way (MDMW)
This interactive Diabetes website helps to support patients with diabetes and their family and friends. The website has an abundance of information about diabetes with leaflets, videos, educational tools and games. You can also access your own NHS clinic results via the MDMW website or App which supports the self-management of your diabetes.
Control IT Plus
This programme is available to those diagnosed with or at risk of developing type 2 diabetes. Control IT is an excellent resource with lots of information to help you understand and manage type 2 diabetes.
If you have not registered with Control IT Plus previously you can self-refer to the programme at any time or speak to your Practice Nurse who can refer you.
